Require: One account per user, users must link PT accounts to their Facebook, Twitter, google, yahoo, disqus, etc. accounts.
The PT Phish forum has been steadily going downhill over the past few years. This is largely, or entirely, due to the current rules and regulations on PT. Or at the very least, lack of enforcement of these rules are the reason why PT has gone downhill.
But there's an easy fix! Take steps that will actually produce results!
Users should get one account on PT and one account only. Allowing multiple accounts has allowed PT to become what it's become - and it's not good.
Require users to link their PT accounts to their Facebook, Twitter, Google, Yahoo, Disqus, or any other accounts that will get rid of the root cause of most problems on the PT forum - anonymity.
The PT forum is huge, but yet it doesn't follow the same management and security structures that make other high profile forums successful. Things could be so much better here.

We’re always open to suggestions for improving PT, but this is not a solution to the stated problem because people are capable of making multiple accounts with these other service providers.
